The Guardian is reporting that Sir Terry Pratchett has “started the formal process that could lead to his own assisted suicide at the Dignitas clinic in Switzerland.”
In connection with the airing of the documentary Terry Pratchett: Choosing to Die, which aired Sunday on BBC2, he announced that he had received the consent forms requesting a suicide from the clinic, and that he plans to sign them soon. “The only thing stopping me [from signing them now] is that I have made this film and I have a bloody book to finish,” he said following a screening at the Sheffield documentary festival Doc/Fest.
Pratchett also said his decision to start the formal process does not necessarily mean he will suicide. According to Dignitas, nearly three-quarters of the people who sign the forms do not go through with taking their own lives.

Given his interest in the subject I thought this might be in his mind. Very, very sad that we are losing such a great writer too early.
